Statewide Protocol Medical and Skill Formulary for SHCGB

1. Equipment services will be required to have:

• Dial a flow device or IV Pumps for Norepinephrine administration
• 100cc NS IV Bags for Magnesium Drips
• D10 Premixed IV Bags for Hypoglycemia no more D50
• Lidocaine Pre-Mix Bag if adding Lidocaine to Service for Cardiac
• Norepinephrine for Septic and Cardiogenic Shock, Epi Drip to remain for Bradycardia (dopamine is no longer part of the minimum equipment list and is not part of these protocols)
• Ibuprofen or Acetaminophen PO Tablet and Liquid
• Reglan
• IV Acetaminophen (Oct 18)

2. Drugs and Skills Optional for Services

• Lidocaine for Cardiac Bolus and infusion
• Pepcid
• Decadron
• Tridal for Pulmonary Edema Only
• Succinylcholine
• DuoDotes Kit ( Declared Nerve Agent Release Only)
• King-LTD Airway-Optional
• LMA
• Alcaine
• Racemic Epinephrine
• Bioshield for OC Decontamination

• Pediatric CPAP
• Surgical Cricothyrotomy-Bougie Assisted
• Xopenex
• Reglan
• Vecuronium
• Ketamine for Pain control or RSI
• Haldol
• Hydrocortisone
• Hydromorphone
• Compazine
• Zyprexa
• Phenylephrine
• Versed 5mg IN
• Glucagon increase to 5mg IV for CCB & BB OD
• Calcium Gluconate
• Pediatric RSI Protocol
